This book was written to stimulate public awareness of a problem which, apparently, only the public can resolve. I have spent considerable time and effort in past years working through the normal channels on what superficially appear to be obscure technical problems of insurance, finance, and hospital organization. The principal conclusion which came to me was that all of those affected by the problem are trapped by the overlapping constraints of a system which, though it has evolved from good motives, was based on two inappropriate premises
